Half price fish permit claim form

Congratulations on your selection as a winner of the 2023 half price Youth Lifetime Fish permit!

A full price lifetime fish permit for youth 15 years of age and under is $616. Your cost in this promotion is $308. If your claim form and payment are not received by June 9th, 2023, the opportunity to purchase a Youth Lifetime Fish permit for half price will be lost and an alternate Nebraska youth will be selected for this half price opportunity. If you would like to pass on this opportunity, please let us know as soon as possible.
